  • by investing (and remaining invested) with Avanteos Investments Limited (AIL), I give my consent to the collection, use and disclosure of personal information as set out in the PDS and CFS Privacy Policy.
  • if my application to become a member is accepted, my membership will be subject to the terms of the trust deed.
  • I understand that any portfolio rebate that may be applicable will be credited to the most conservative option in my portfolio.
  • I understand that other than FirstRate products, investments in FirstChoice Wholesale Personal Super options are subject to investment and other risks, including fluctuation in value, possible delays in repayment and the loss of income. The repayment of capital of this product is not guaranteed. 
  • I acknowledge that I am unable to access my account balance until a condition of release is met (as set out in the PDS).
  • I’ve downloaded the PDS and understand that it is important to consider before deciding whether this product is right for me.
  • I confirm I have legal power to invest and acknowledge the risk and return characteristics for each selected investment option. For more information, please refer to the Investment Options Menu.
